How to Create a Dynamic Form

Build custom forms and HOPs for engineers to complete on site via the XMP mobile app.

Overview

Dynamic Forms let you replace paper-based and Excel-based documents — handover packs (HOPs), photo sheets, end-of-job checklists — with digital forms your engineers fill in directly from the XMP mobile app on site. Responses are captured in XMP instantly, cutting out hours of manual data entry back in the office.

Creating a New Form

  1. Go to Main Menu and scroll to the bottom, then click App Configuration -> Dynamic Forms
  2. Click "Add New"
  3. Give the form a name (e.g., "BT Photo Sheet" or "End of Job Checklist")
  4. Assign it to a category to keep your forms organised (these are set Company HR settings > Dynamic Forms Categories)

Adding Sections

  1. In the form builder, click "Add Section" on the left panel
  2. Give each section a descriptive name (e.g., "Site Details", "Installed Equipment", "Sign-Off")
  3. Add as many sections as the form requires. Note - each section requires at least one question.

Adding Questions

  1. With a section selected, click "Add Question"
  2. Enter the question text (e.g., "Site ID", "Length of feeder installed (m)")
  3. Click the Answer tab to choose the answer type:
  • Text — free-text response
  • Photos — engineer uploads photos from their device or camera
  • Multiple Choice — provide a list of options for the engineer to select
  • Update Milestone — the engineer's answer records or updates a milestone status on the job (e.g., "DRA Completed")

Reusing and Duplicating Forms

If you need client-specific variations of the same form, duplicate an existing form and adjust the details. Engineers assigned to a job can access all applicable forms via the XMP mobile app.

Availability

Forms are available to engineers on the XMP mobile app. By default, all forms are available to all engineers.

To restrict access to a form, you can assign it to a specific site, project, or work type. This is done in Availability options by setting up a conditional group.

Responses

Form responses are captured in XMP instantly, directly from the XMP mobile app and can be viewed from Main Menu -> Forms

Responses can be viewed from XMP, downloaded as a PDF, or exported as an Excel file.
